Dr. Saif Choudhury is Impact Physician Group’s Vice President of Skilled Nursing Operations. With a drive to improve quality for every patient, Dr. Choudhoury has a heart warming personality and has maximum time for every patient. As a teacher and physician, he sets the tone for Impact Physician Group providers as they focus on the patient’s current status and potential for improvement.

Dr. Choudhury is a 2005 University of Chicago science graduate who completed his Doctorate of Medicine at the America University of the Caribbean-School of Medicine in St. Maarten, Netherlands Antilles in 2010. In his residency at Hennepin County Medical Center in Minneapolis, he distinguished himself as Chief Resident. He is board certified by the American Board of Family Medicine (ABFM).

In his staff time at Froedert & Medical College of Wisconsin-St. Joseph’s Hospital he achieved the highest patient satisfaction scores. In subsequent work with hospitalists (MD and Nurse Practitioners) he has led the demonstrated achievement of a 50% reduction of hospital readmissions as measured against national benchmarks.
